to thse measures the Division requires that the applicant provide the following: a) a <br />representative cross sectional design of a typical monitoring well installation, b) a commitment to <br />provide the Division with copies of the well completion reports following installation, c) a <br />commitment to monitor the groundwater levels on a daily basis for a period of at least 30 days <br />upon initiation of pumping from the excavated pit dewatering trench, d) a commitment to notify <br />the Division within 24 hours, or the next working day, if there is a significant change in the <br />groundwater level detected at any of the five monitoring wells during the monitoring program, <br />and e) a commitment to provide the Division with the previous year's groundwater monitoring <br />data and a summary of results in the applicant's Annual Report. Please respond. <br />19. Please commit to providing the Division copies of correspondence from the Office of the <br />State Engineer verifying an approved temporary substitute supply plan and issued well permit <br />prior to exposure of groundwater at the Nissen Farm Resource site. <br />6.4.12 EXHIBIT L -Reclamation Costs <br />21. Hall-Irwin has stated that in their response to Item 3 that they will submit the necessary <br />design specifications for the slurry wall installation as a technical revision prior to exposure of <br />groundwater. This implies that the design specifications, pit floor evaluation, construction <br />report, and QaQc plan will not be submitted until after construction of the slurry wall. Under <br />these conditions, the Division will require a 100% bond for slurry wall installation. At this time, <br />the Division will only accept a 20% bond for slurry wall installation if the applicant provides a <br />definite commitment to provide the Division with the necessary design specifications, pit floor <br />geologic evaluation, and QaQc Plan as a technical revision prior to installation of the slurry wall. <br />Please respond. <br />6.4.14 EXHIBIT N -Source of Lesal Right to Enter <br />24. Based on the revised legal description for the project boundary as provided on July 31, 2003, <br />please affirmatively state that Hall Irwin is, or is not, applying for a mining and reclamation <br />permit for Lot B only and not any other parcels. <br />25. The applicant's legal right of entry must be based on the exact location of all gas wells and <br />appurtenances, gas lines, flow lines, tank batteries, etc. The approximate location of such <br />structures is insufficient for establishing the applicant's legal right of entry. Please verify that the <br />applicant's legal right of entry is based on the exact location of all gas wells, appurtenances, gas <br />lines, flow lines, tank batteries, or other such structures; or the exact location of the easement <br />boundaries for such structures. <br />6.4.19 EXHIBIT S -Permanent Man-made Structures <br />26.
