-Show the name of the applicant <br />-be prepazed and signed by a qualified person <br />-give date prepared <br />-identify and outline azea which corresponds with the application <br />-prepazed at a scale that is appropriate to clearly show all elements that aze <br />required .The acceptable range of map scales shall be no lazger than 1 inch=50 <br />feet nor smaller than 1 inch= 600 feet. Also, that a map scale, appropriate <br />legend, map title, date and a north arrow shall be included. <br />The various maps included typically show one or more but no[ all of the <br />required elements. The application should include the following labeled maps. <br />Index map, Mine Plan Map, and Reclamation Plan Map. Following is a list of <br />inadequacies for the application maps. <br />Index map- The county map and the map drawn on graph paper are adequate <br />to locate the site, however, the prepazed map should also be titled, include a <br />scale and show names of all land owners within 200 feet of the affected area. <br />Mine Plan Map- This map must be titled, include the owners of land within <br />200 feet of the affected azea, identify owner of substance to be mined and <br />identify owners of structures within 200 feet of affected area boundaries. In <br />addition to the features already included (topsoil and product stockpiles) the <br />map should also include stream channels, impoundments and diversions and <br />indicate the direction that material extraction will proceed. <br />Reclamation Plan Map- This map must be titled and show the gradient of all <br />reclaimed slopes. In addition, it must state the thickness of replaced <br />overburden and topsoil. <br />Please remember to include dimensions of the proposed mining area as well as <br />an appropriate scale on all maps. <br />6.3.6 EXHIBIT F -List of Other Permits and Licenses Required <br />Enclosed for your review aze any comments that we have received from other agencies <br />that DMG solicits for comment on new applications. One agency that you will need to <br />contact is The Water Quality Control Division of the Colorado Department of Health and <br />Environment to determine whether or not you will be required to obtain storm water <br />permits for your site. Their contact information is (303) 692-3606 or 692-2000. <br />6.3.7 EXHIBIT G -Source of Legal Right-to-Enter <br />This section is adequate as submitted. <br />
