Banta <br />Dnis~an D~re•_1o~ <br />The following items must be addressed by the applicant in order to satisfy the <br />applicable requirements of C.R.S. 34-32-101 et seq. and the Mineral Rules and <br />Regulations of the Mined Land Reclamation Board- <br />1. It appears that access to this site will require crossing Sneffles <br />Creek. To prevent undue increases in the sediment level of the creek due <br />to vehicle traffic, please commit to protecting the creek bottom with <br />rocks of 6 to 8 inches in diameter at the point where crossing will <br />occur. You should try to limit the amount of rock placed in the creek <br />crossing area to less than 200 cubic yards; however, if more is required <br />to elevate traffic out of the creek bottom, then you should contact <br />Mr. Grady McNure at the U.S. Army Corps of Engineers office in <br />Grand Junction, Colorado to discuss the necessity of a 404 fill permit. <br />Please also conmit to removing this rock from the creek bed once mining <br />has ceased. <br />2. Is any local government approval necessary for your proposed operation, <br />and if so, have you already applied for it? <br />3. Are you the owner of the surface of the affected land? If not, please <br />provide the name, address and telephone number of the owner(s). <br />4. Are you the owner of the subsurface rights of the affected land? If not, <br />please provide the name, address and telephone number of the owner(s). <br />5. Please provide more information describing your mining operation, <br />including any necessary portal and adit reinforcement, mining tools and <br />equipment, and installation or improvement of ore car rail. <br />6. What are your plans for reclamation of the portal? Options include <br />backfilling the opening shut, sealing it shut with rock and cement, or <br />installing a locking door. <br />7.
