Mr. lames Striggow, Glade, LLC <br />Adequacy Review, Pemut No. M-2006-021 <br />June 22, 2006 <br />5. The Township and Range on both Mining Plan Maps of Exhibit D appeaz to be mislabeled. Please verify the <br />Township and Range and revise Exhibit D to reflect the proper Township and Range. <br />Rule 6.4.4(fl(i) Exhibit D - Minine Plan <br />6. )n accordance with this rule, please indicate the depth of the sandstone to be mined. <br />Rule 6.4.5(2)(fl(i) Exhibit E -Reclamation Plan <br />7. In accordance with this rule please specify the maximum anticipated slope gradient or expected range of slopes. <br />Rule 6.4.5(2)(fZ(, Exhibit E -Reclamation Plan <br />8. Tn accordance with this rule, please specify anticipated minimum depth or range of depths for those areas where <br />topsoil will be replaced. <br />Rule 6.4.6(a) Exhibit F -Reclamation Plan Ma-p <br />9. The proposed post-mining topography is not clear on this map. In accordance with Rule 6.4.6(a), the map must show <br />proposed topography of the area with contour lines of sufficient detail to portray the direction and rate of slope of all <br />reclaimed lands. Please provide a reclamation plan map that meets these criteria. <br />10. The Township and Range on the Reclamation Plan Map of Exhibit F appears to be mislabeled. Please verify the <br />Township and Range and revise Exhibit F to reflect the proper Township and Range. <br />Rule 6.4.7 (3) and (4) Exhibit G-Water Information <br />11. In accordance with these two rules please provide an estimate of the project water requirements and the sources of <br />water supply to meet these requirements. <br />Rule 6.4.14 Exhibit N -Source of Leal Right to Enter <br />12. The lease agreements provided as documentation of Legal Right to Enter indicate that the Legal Right to Enter <br />belongs to Stone Wholesale Corporation rather than Glade, LLC. Please provide documentation that Glade, LLC has <br />Legal Right to Enter. <br />Rule 6.4.18 Exhibit R -Proof of Filing with County Clerk and Recorder <br />13. The receipt indicating that the application was placed on file with the local County Clerk and Recorder for public <br />review does not indicate the date on which the application was filed. In accordance with Rule 6.4.18, please provide a <br />receipt indicating the date that the application was filed with the local County Clerk and Recorder. <br />Rule 6.4.19 Exhibit S Permanent Man-made Structures <br />14. As described above in Item No. 3, numerous permanent man-made structures were observed within 200 feet of the <br />affected area. In accordance with Rule 6.4.19, please indicate if the mining operation will adversely affect the stability <br />of those structures and if so, please address this issue as indicated in Rule 6,4.19(a) thru (c). <br />Enclosed aze copies of comments received from the Army Corps of Engineers and the Colorado Historical Society. <br />Please be sure to address any issues of concern with these offices directly. <br />
