Dear Mr. Striggow,
Listed below are adequacy review comments on the proposed Glade Quarry Permit Application No. M-2006-021. The
Division of Minerals and Geology (Division) is required to make an approval or denial decision no later than July 20,
2006, therefore, your response to the following adequacy review concerns should be submitted to the Division as soon as
possible.
Rule 6.2.1(2) General Requirements -Mans and Exhibits
1. All maps (other than the indexllocation map) must show the name of the Applicant; must be prepared and signed by a
registered land surveyor, professional engineer, or other qualified person; and give the date prepared. Please revise
Exhibit C -Pre-mining Plan Map, Exhibit D -Mining Plan Maps, and Exhibit F, Reclamation Plan Map to include
this information.
Rule 6.4.1 Exhibit A -Legal Description
2. The Township and Range of each area to be mined appear to be mislabeled. Please verify the Township and Range
for each section to be mined and submit a corrected Legal description if necessary.
Rule 6.4.3 Exhibit C -Pre-Mining and Mining Plan Map(sl of Affected Lands
3. Rule 6.4.3(g) requires the applicant to show the owners name, type of structures, and location of all significant,
valuable, and permanent man-made structures contained on the area of affected land and within two hundred (200)
feet of the affected land. During the pre-operation inspection conducted on April 26, 2006, numerous man-made
structures were observed within two hundred feet of the affected area. Theses structures include several irrigation
ditches and associated culverts and headwalls on the east side of the hogback; several barbed wire fences running
east-west on both sides of the hogback; a potential underground cable running north-south on the west side of the
hogback; an earthen berm at the south end of the site; and a PVC pipe running east-west on the ground surface at the
south end of the site. Please revise Exhibit C -Pre-mining Plan Map to show the owners name of these structures
observed on the site.
4. The Township and Range on Exhibit C -Pre-Mining Plan map, and in the Pre-Mining Plan Notes of Exhibit C appear
to be mislabeled. Please verify the Township and Range and revise Exhibit C to reflect the proper Township and
Range.
