i • <br />* They must be signed by the certified blaster conducting the blast, and show his or her name <br />and certification number. <br />* They must identify the nearest dwelling, public building, school, church, community or <br />institutional building outside the permit area and establish the direction and distance from the <br />nearest blast hole to the nearest structure. <br />* They must note weather conditions, including those which may cause possible adverse <br />blasting effects. <br />* They must record the type of material blasted. <br />* They must incorporate sketches of the blasting pattem, including number of holes, burden, <br />spacing, decks and delay pattem. <br />* They must specify the diameter and depth of holes, and the type and length of stemming. <br />* They must record the types of explosives used, the total weight of explosives detonated in any <br />one 8 millisecond period. <br />* They must specify the initiation system. <br />* They must provide details of mats or other protections used. <br />If Agile Stone Systems decides to conduct vibration monitoring, then the following information must <br />also be included into the records. <br />* Type of instrument; sensitivity (or "range setting") and calibration signal or certification of <br />annual calibration. <br />* Exact location of instrument, and the date, time and distance from the blast. <br />* Name of the person and firm (can be the operator) taking the reading. <br />* Name of the person and firm (can be the operator) analyzing the seismographic record. <br />* The vibration level and/or airblast recorded. <br />Please call if you have questions. <br />Sincerely, <br />Larry D.
