As I <br />discussed with you by phone today, there are 6 issues that we request you address as soon as possible. This <br />can be done by committing to incorporate the following list of items into your permit application. <br />1. Agile Stone Systems will mazk the granite and sandstone quarry sediment control ponds to show <br />where the 50% sediment storage azea will be and clean the ponds when they reach that level, prior to <br />adding additional sediment. <br />2. Within 60 days of permit issuance, Agile Stone Systems will provide a written plan for testing various <br />mixtures of plant fines and topsoil as a reclamation plant growth medium. The location of the test <br />plots will be marked on the permit mining plan map. The most suitable mixture will be used in <br />accordance with the granite quarry reclamation plan. <br />3. Topsoil stripping will be limited to two (2) bench widths in the granite quarry and 100 feet in advance <br />of gravel extraction in the gravel mining areas. <br />4. Within 60 days of permit approval the reclamation plan map will be modified to show the proposed <br />topography of the permanent visual noise berms. <br />5. The financial warranty (reclamation bond) shall be set at $276,700.00. <br />6. Blasting will be conducted in such a manner that airblast levels at the closet off-site residential <br />structures will be 134 decibels or less. Blasting records will be kept for 3 years for each quarry blast <br />and will be available for inspection by the Mined Land Reclamation Division.
