Phase IV Weekly Summary No. 33 Page 2 003-2203 <br />squaze feet (total) of 80-mil textured LLDPE in the LOB area. <br />• Ames began placement of Drain Cover Fill (DCF) on September 20a' down the ramp on the East side <br />of the LOB area. Drain Cover fill was hauled to the LOB area using CAT 777 haul trucks and <br />placed with a CAT D6M. <br />No confirmation drilling was performed in this area during this reporting period. <br />Phase IVc <br />Ames began confirmation drilling at UG-242. No workings were located. <br />Ames did not perform any cut or fill activities within the Phase IVc azeas during this reporting <br />period. <br />(See Attachment A for approximate construction locations for all Phase IV azeas.) 1 <br />Field Monitorin¢ and Testin¢ Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials were verified as suitable and construction methods were in accordance <br />with project specifications. Removal of oversized and deleterious material from all fill azeas was <br />observed. <br />Golder observed and recorded confirmation drilling and remediation of underground workings in the <br />all Phase IV azea during this reporting period. Confirmation drilling was observed full time. <br />Golder monitored and documented geomembrane installation. Golder observed panel deployment, <br />including panel thickness measurements and mazking defects. Golder monitored and documented <br />trial seam tests, seaming, seam pressure tests, vacuum testing, patching, and identified fusion seam <br />destructive samples and extrusion seam destructive sample for testing by Polyflex. <br />Golder provided full time observation of DCF placement to verify that the material was placed a <br />minimum of two feet thick, geomembrane wrinkles were not folded, oversized and deleterious <br />materials were not present in the fill, and HVSC pipes were kept in place and haunched as needed. <br />Laboratory Test Results <br />No laboratory testing was completed during this reporting period. <br />Meetings <br />CC&V held a Weekly Contractor meeting on September 18°i. Items discussed include Safety, <br />Environmental, Schedule, and CQA.
