Newport St. <br />Centermial, CO 80111 <br />Re: Byzantine Quarries, File No. M-2007-014, New 112(c) Application, Permit No. M- <br />2007-014; Adequacy Review -Additional Information Requested <br />Deaz Mr. Clazk: <br />Listed below are the additional items and information we request in order to complete our <br />adequacy review on the Byzantine Quarries, New 112(c) Application, Permit No. M-2007-014. <br />The Division of Reclamation, Mining, and Safety (Division) is required to make an approval or <br />denial decision no later than 3une 22, 2007, therefore, your response to the following adequacy <br />review concerns should be submitted to the Division as soon as possible. Your response should <br />include two (2) copies. Any public comments that we receive on your application during the <br />interim will be forwazded to you. <br />1. As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public <br />notice circulated in the local newspaper, all owners of record of surface and mineral <br />lands, easement holders, and owners of record of surface lands that aze within 200 <br />feet of the boundary of the affected land. Proof of notice may be return receipts of a <br />Certified Mailing or by proof of personal service. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional <br />engineer, or other qualified person. The maps submitted to the Division were not <br />signed. Please resubmit a signed copy of the maps. <br />Rule 6.4.3 Exhibit C- Pre-Mining and Mining Plan Map(s) of Affected Lands <br />3.
