time augmentation will occur (ie: until mine areas 2 and 3 are fully bacldilled to approved post-mining <br />topography, until premising groundwater levels aze observed, etc.). <br />Stioulatkon No. 5 <br />The division accepts the assumptions used in developing the slope stability analysis with the following conditions: <br />1. Develop a method to measure and report to the division how slopes are being maintained at a minimum of <br />2H:1V during mining. Those values must ba measured quarterly and submitted to the division with the <br />required amual report. If slopes are determined at anytime to be steeper than 2H:1 V, the division must <br />be notiSed immediately. Tn addition, DMG must be notified when mining is to occur within 200 fed <br />of any structure for which this or future stability analyses are provided. The office will then comdud <br />an inspection ro verify that slopes era being maiinained at 2H:1 V or flatter and to inspect the area above <br />cut slope for tensics cracks or slumping. <br />2. If cracking or slumping is observed within the 25 foot buffer of nay sttucarre covered under a stability <br />analysis, miming must cease and the division must be notiSed immediately. <br />3 . If trading or slumping is observed at anytime above any cut-slopes, the division must be notiSed and <br />the stability of the system must be re-evaluated prior to continued mining activities. <br />The a~ of fmaaaal warranty set by the Divisim for this operatics is $3I 7,000.00.
