<br /> <br />-2- <br />The Division is reminding all permit applicants that all permit <br />holders are required to mark the boundaries of the affected area on <br />the ground. You are also required to place a sign at the entrance <br />to the permitted site which includes; site name, operator name, <br />statement that the site is permitted with MLRD and the permit <br />number. <br />In Exhibit D, Reclamation Plan, please specify the volumes of <br />material (topsoil, overburden, gravel, etc.) that will be moved to: <br />(a> create the earth dam, ponds and open pit and (b> reclaim the <br />earth dam, ponds, open pit, embankments and surface facilities. <br />Rule 2.1.2(5)(e) requires that you provide an approximate timetable <br />to describe the mining operation. The timetable is for the purpose <br />of establishing the relationship between mining and reclamation <br />during the different phases of a mining operation. The timetable <br />shall include: (i) an estimate of the periods of time which will <br />be required for the various stages or phases of the operation; (11) <br />a description of the size and location of each area to be worked <br />during each phase; and (iii) outlining the sequence to which each <br />stage or phase of the operation will be carried out. Specify <br />maximum disturbed acres as associated with this timetable. <br />To adequately review your responses to the adequacy review, the Division must <br />receive the required information by May 15, 1992. If we do not receive ail <br />the required information by that date, a new date may be set for consideration <br />of the application as provided for under Rule 1.5(2). Also, as provided for <br />under Rule 1.5.1 all changes and additions to the permit application must be <br />filed with the Division, and with the County Clerk and Recorder. The <br />applicant shall also provide the Division with an affidavit or receipt <br />demonstrating that the change was filed with the County Clerk and Recorder no <br />later than the close of business on the day the change was filed with the <br />Division. Any change or addition to an application shall constitute an <br />amended filing for purposes of detemining the date for the consideration of <br />the application by the Division, and for the deadline for a final decision on <br />the application.
