3. Noncompliance Voci_fication (continued)
b) The permictee shall report to following instances of
noncompliance orall`r within caenty-Four (2~, hours from the time
the permittee becomes aware of the noncompliance, and shall mai:

becoming aware of the noncompliance:
(i) any instance of noncompliance which say endanger health or
the environment;

(iii) Any upset which causes an exceedance of any effluent
limitation in the permit;
(iv) Any spill which causes any effluent limitation to be
violated;
(v) Daily maeimum violations for any toxic pollutants or
hazardous substances limited by PART I-d of this permit and
' specified as requiring 24 hour no cificacion. .
