(25-7-101) <br />se 1, TO THOSE GENERAL TERMS AND CONDITIONS SET FORTH ON THE REVERSE SIDE OF THIS <br />DOCUMENT AND THE FOLLOWING SPECIFIC TERMS AND CONDITIONS: <br />1. The permit number shall be marked on the subject equipment for ease of identification. (Reference: <br />Reg. 3, Part B, IV.E.) (State only enforceable) ;. <br />2. Conveyors shall be equipped with 3/4-covers to minimize dust emissions. <br />3. Drop points shall be fully or partially enclosed to minimize dust emissions. <br />4. In accordance with C.R.S. 25-7-114.1, the Air Pollutant Emission Notice (APEN) associated with <br />this permit is valid for a term of five years. The five year term for this APEN expires on August 18, <br />2001. A revised APEN shall be submitted no later than 30 days before the five year term expires. <br />5. Construction of this source must commence within 18 months of initial approval permit issuance <br />or within 18 months of the start-up date stated in the application. If commencement does not <br />occur within the stated time the permit will expire on 1!IARCB 23, 2000 <br />(See General Condition No. 6., Item 1 on the reverse side of the first page of this permit.) <br />6. This permit replaces permits 13GU034-1 and 88GU038F. <br />7. Each time this equipment is moved to a new location within the State of Colorado the owner or <br />operator shall file a Relocation Notice. Such notice shall be received by the Division at least ten <br />(101 days prior to the change in location. The Relocation Notice shall include a facility emission <br />inventory of all emission units at the site. (Reference: Regulation 3, Part A,II.C.1.f and Part B.IV.E.) <br />• 051 /0015/011 <br />
