Long <br />Division Director <br />The Division has reviewed your permit application fora 112 sand and gravel operation File # M- <br />2002-009, had has the following comments. <br />EXHIBIT "A" , LEGAL DESCRIl'TION (6.3.1) <br />1)1n Exhibit "A" General Description and Exhibit "B"Index Map, the site appears to be located <br />in section 19, however, the detailed narrative description describes the pazcel as being located in <br />section 18 beginning at the North East corner. Please explain <br />2) In addition, the index map shows all of the permit area to be within section 19, however, the <br />pre- mine and mine plan map shows a portion of the northern boundary to cover a small area of <br />section 18 ,please clazify. <br />EXHIBTI' "B" INDEX MAP (6.3.2) <br />1) Please refer to comments under Exhibit "A" <br />EXHIBTI' "C" Pre- Mining and mining plan maps of affected lands (6.4.3) <br />1) The rule requires the operator to show the owner's name, type of structures and location of all <br />significant, valuable and permanent man made structures contained on the azea of affected land <br />and within two hundred (200) feet of the affected land.
