Phase IVc <br />• Ames placed Structural Fill (SF) in the Phase Nc area at the saddle berm neaz UG-242. Structural Fill <br />materials were placed in horizontal liRs typically not exceeding 2 feet in loose thickness. A Caterpillar <br />CS563C roller provided compactive effort, while a Caterpillar water wagon provided moisture <br />conditioning when needed. Ames conducted slope recontouring in the Phase Nc area below the 9724 <br />road at station 7+00. Ames used Structural Fill to backfill over remediated "Deep" workings UG-246b <br />and UG-521. <br />Ames performed excavation remediation activities of known underground working UG-253. This <br />working was observed and verified to be a "Deep" as defined in the drawings and will be remediated <br />accordingly. <br />Ames performed confirmatory drilling activities associated with the Phase IYc construction azea on <br />known underground workings UG-495 and UG-498. <br />Ames placed Cemented Rockfill (CR) at UG-257 and UG-301 by tapering the working and placing 7 <br />feet of CR in accordance with IFC Drawing 12, Detail 1. <br />Ames constructed a geogrid cap over UG-257. Ames placed two layers of geogrid in accordance with <br />the drawings and specifications. <br />Ames continued blast drilling of underground workings within Phase Nc at UG-270. Ames blasted <br />underground workings delineated by confirmation drilling at UG-270 on May 2nd at 12:00. <br />• (See Attachment A for approximate construction locations for all Phase IV areas.) <br />Field Monitorine and Testine Activities <br />Golder observed construction activities associated with Phase N earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. Golder conducted temperature measurements of fill material prior to placement. <br />Golder observed recontouring of the Phase III ore slope. Ore was regraded to approximately 4H:1 V <br />and at no time was allowed off of the existing liner system. <br />Golder observed construction of the Primary Underdrain and Secondary Underdrains in Phase IVb <br />for trench dimensions (5' by 3' and 4' by 2'), slope (minimum 1%), geotextile (12 oz.), UF, <br />geotextile overlap (2'), and SSF. <br />Golder observed fill placement at the 84-inch culvert inlet. SSF and SF was placed in approximately <br />1-foot lifts and compacted in accordance with the specifications. <br />Golder observed and tested placement of SLF at the LOB completing three in-situ density tests, three <br />depth verification tests, and verifying surface preparation.
