<br />lands. If a release does occur, it shall be reported to this office immediately. The permittee is <br />responsible for disposing of all debris in accordance with state and federal regulations. <br />5. Distwbed areas will be monitored for noxious weeds for three (3) years following the <br />revegetation activities. <br />6. Control of noxious weeds will be required through successful vegetation establishment and/or <br />herbicide application. If applications of herbicide are prescribed, however, it is the responsibility <br />of the lease operator to insure compliance with all local, state and federal laws and regulations, as <br />well as labeling directions specific to the use of any given herbicide. <br />A Pesticide Use Proposal (PUP) will be approved prior to application of herbicides and/or other <br />pesticides on Federal surface; contact the Little Snake Resource Area office to obtain a PUP <br />form to request this authorization. Submit the PUP 2 months in advance of planned application. <br />In the event you elect to apply herbicide or other pesticide as described and authorized on the <br />approved PUP, you must report this use within 24 hours on Bureau of Land Management form <br />titled Pesticide Application Record. <br />All equipment and machinery shall be equipped with spark arresters and mufflers. <br />8. Pertnittee is responsible for all suppression costs for any fire resulting from their operations and <br />practices. <br />9. Topsoil will be stockpiled for pit rehabihtation measures. <br />10. All pits shall be marked as Moffaz County material sites with Colorado Mine Land Reclamation Board <br />permit signs. <br />11. "No dumping" signs will be erected at the site. <br />l2. Warning signs shall be placed where appropriate during operational phases to minimize hazards to <br />passing traffic. <br />13. Between operations, the pit walls shall be maintained at a slope ratio not to exceed 3h: I v, so as to <br />minimize slope failure potential and pubhc safety hazards. <br />14. Yearly production Sgttres ~'or the pit shall be reported to the Little Snake Field Office by no later than <br />March 1st. <br />15. Assignment of materials from this pit for other than Moffat Counry purposes shall be made only after <br />notification and approval from the Bureau of Land Management, Little Snake Field Office. <br />16. To eliminate disturbance of existing power, telephone, and road rights-of--way, there will be no <br />excavation within 40 feet of power poles, telephone poles, pole anchors, or the nearest side of an <br />existing road. v. <br />17.
