J
your office on 11-21-2002 indicated that no such application has been received by their
office. Please verify that the well permit application has been submitted to the SEO.
Exhibit H -Wildlife Information
33. Please address the concerns listed in the enclosed letter from the Division of Wildlife.
Exhibit I -Soils Information
34.According to the soil classification information, it appears that the Green River Very Fine
Sandy Loam that will be removed from Phase 1 and placed on the Staging Area may be the
best growth medium available on site when the limited amount of Ravola Very Fine Sandy
Loam is expended. Please address the suitability of this soil type to establish and maintain
plant growth.
Exhibit J -Vegetation Information
35.Please verify that there are no jurisdictional wetlands located within the proposed affected
area. The verification should be completed by the Army Corps of Engineers or a qualified
consultant.
36.It is recommended that the permit applicant contact NRCS to determine relation of
vegetation to soil types and topography of the site.
Exhibit L -Reclamation Costs
37. Please revise the Reclamation Cost exhibit to include the revisions required by this adequacy
review. If disposal of asbestos and other wastes become part of this application, then
reclamation costs for those tasks may need to be incorporated into this estimate. This may
cause the initial phases of the operation to be the most costly to bond for. Documentation
of the amount of material to be disposed of and costs for that type of disposal would need
to be submitted.
38.A reclamation cost estimate for this operation will be calculated upon receipt of responses
to this adequacy review.
Exhibit M -Other Permits and Licenses
39.This exhibit lists permits and licenses required for this operation. However, each of the
items in this exhibit must indicate if they have been obtained or have been applied for.
Please revise the exhibit accordingly and re-evaluate whether or not an Army Corps of
Engineers Permit is required.
Exhibit S -Permanent Man-made Structures
40.There is a U. S. West (Qwest?) easement shown of the application maps. Please verify if
there is a structure in this easement belonging to U. S. West (Qwest?I. If so, please
adequately address this structure as required by Rule 6.4.7 9 and include the structure on
the permit maps.
