Please revise the Reclamation Plan Map to incorporate the required revisions noted in the <br />section above, and include a north arrow. <br />Exhibit G -Water Information <br />25.Please include all of the various wells and piezometers on and within 200 feet of the <br />proposed permit area on the Pre-mine, Mine Plan, and Reclamation Plan maps. <br />26.Please show and denote the proposed site perimeter berm, to route upland drainage around <br />the site and contain stormwater that falls on the site, on the Mine Plan and Reclamation <br />Plan Maps. <br />27.The Mine Plan Map indicates that the 100 year flood will cross portions of the Phase 5 area. <br />Therefore, the statement "The lake created from the mining operation is not in any danger <br />of being captured by the river during a flood" is not accurate. The lake may temporarily be <br />captured during the flood. Please clarify this discrepancy. <br />28. Because of the potential for hazardous and/or toxic materials to reach the sediment pond, <br />please commit to using and maintaining a valve that is water tight on the outlet structure <br />and provide detailed specifications for the outlet installation that include measures to <br />prevent piping or other bypass of the outlet pipe. <br />29.CDPHE has addressed handling of potentially contaminated soils excavated during <br />construction of the sediment pond. Please indicate what measures will be taken to ensure <br />the introduction of water to the sediment pond will not mobilize toxic or hazardous materials <br />from the soil into the groundwater system or into the introduced water. <br />30. Please specify what safeguards will be in place to ensure dewatering of the pit does not <br />cause the sediment pond to discharge unintentionally. <br />31.The pit dewatering discharge location (outfall 001) evidently changed, from the location <br />specified in the NPDES Permit included with this application, when the sediment pond <br />location was revised. Please specify where the new outfall 001 is and show this location <br />on the permit maps. One map indicates that this point may be at the outlet of the ditch to <br />Reed Wash. If this is the case, please indicate measures to be taken to differentiate <br />between contamination from the permit area and contamination from stormwater runoff <br />that enters the ditch from lands adjacent to the permit area. <br />32.The application indicates that a well permit for water consumption by this operation has <br />been applied for.
