D) A monitoring plan that will ensure that treated areas receive follow-up site inspections and secondary <br />treatment as necessary, and that the remainder of the site is being monitored on a regular basis for early <br />detection and treatment. <br />Condition #3: Map of the Permit Area <br />The applicant will submit a single map clearly illustrating the extent of the proposed affected or <br />disturbed area within the boundaries of the permitted area. This should include all areas ancillary [o the <br />development of the Phase I mining area, including but not limited to, roads, rail lines, ponds, topsoil stockpile <br />locations, plant development areas, surface water control stmctures, pit development, etc. <br />Condition #4: Hazardous Materials Spills <br />A reportable spill is a spill of any toxic or hazardous substance (including spills of petroleum products) <br />within the permit area reportable to any division of the Colorado Department of Public Health and the <br />Environment, the National Response Center, the Colorado Emergency Planning Commission, a local emergency <br />planning commission, a local emergency planning committee, or the State Oil Inspector. <br />The operator shall notify the Division of a reportable spill within the permit area using the same <br />timeframe required by the permit, license, notice, act, rule or regulation governing the reporting of the spill to <br />the appropriate agency. Notice of a reportable spill shall be faxed to: Minerals Program Supervisor, Division of <br />Minerals and Geology, FAX (303 832-8106), The FAX shall include acal]-back number of a responsible <br />company official for DMG staff to use as a contact. <br />Condition #5: Reclamation Droueht Plan <br />GCC Rio Grande will develop a "Reclamation Drought Plan" and submit it to the Division within 90 days of <br />permit approval. The Reclamation Drought Plan shall be approved by the DMG prior to any disturbance <br />occurring on the site (except for surveying/testing and water resource development), including mining and <br />constmction of any portion of [he facility. <br />]8 <br />
