A copy of the SWMP shall also be provided to the municipality upon request. <br />S. Special Monitorine <br />a. TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as electrica <br />conductivity where a satisfactory correlation with TDS has been established based upon a minimum offive (5) samples. Sec <br />the individual Certification Rationale for a determination as to whether monitoring is required. <br />b. Phosphorus monitoring is required only for facilities which discharge into the following drainage bazins: Cherry Creel' <br />basin, Chatfield Reservoir downsaeam of the USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e„ Tez <br />Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the indi«du <br />Certification Rationale for a detemunaaon as to whether monitoring is required. <br />9. SienatorvReq_uirements <br />a. All reports required for submittal shall be signed and certified for accuracy by the pemrittee in accordance with the followir~ <br />criteria: <br />1 } In the case of corporations, by a principal executive olcer of at least the level of vice-president or his or hez du' <br />authorized representative, if such representative is responsible for the overall operation ofthe facility from which d i <br />discharge described in the form originates; I <br />2) In the case of a parhtenhip, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor, <br />4) In the case of a mtmicipal, state, or other public facility, by either a principal executive officer, ranking elect' <br />official, or other drily authorized employee. <br />b. Changes to authorization. If an authorization under paragraph a) of this section is no longer accurate because a differ <br />individual or position has responsibility for the overall operation of the facility, a new authorization satisfying <br />requirements of paragraph a) of this section must be submitted to the Division, prior to ar together with any repo <br />information, or applications to be signed by an authorized representative. <br />c. Cepcation. Any person signing a document under paragraph a} ofthis section (except for the SW7vfP certification} <br />make the following certification: <br />"I certify under penalty of law that this document and all attachments were prepared under my direction or supervisit <br />accordance with a system designed to assure that qualified personnel properly gather and evaluate the information subm <br />Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gatherir <br />information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete.
