Canany <br />Division DireROr <br />RE: Lafarge West, Inc., Application No. M-2003-091 Natural Resource Tmstee <br />Duckworth Pit; Adequacy Review <br />Dear Mr. Reckentine, <br />Listed below aze adequacy review comments on the Duckworth Pit New 112 Application, File <br />Number M-2003-091. The Division of Minerals and Geology (Division) is required to make an <br />approval or denial recommendation no later than March 31, 2004, therefore, your response to the <br />following adequacy review concerns should be submitted to the Division as soon as possible. <br />As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public notice <br />circulated in the local newspaper, all owners of record of surface lands, easement holders, <br />and owners of record of surface lands that are within 200 feet of the boundary of the affected <br />land. Proof of notice may be return receipts of a Certified Mailing or by proof of personal <br />service. This includes owners of structures within 200 feet of the affected land (i.e. wells, <br />waterlines, WCR 20 %z, ditches, etc...) <br />Rule 6.4.3 Exhibit C-Pre-Minin¢ and Mining Plan Man(sl of Affected Lands <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional <br />engineer, or other qualified person. The maps submitted to the Division were not signed. <br />Please resubmit signed copy of the maps. <br />3. Lafazge identified two adjacent landowners as Cooley Gravel Co. and Western Paving <br />Construction Co. Both of these companies have merged, the first to Aggregate Industries, <br />WCR and the second to Lafarge West, Inc. The maps should be revised to reflect these <br />current landowners. In addition, please assure the Division that Aggregate Industries, WCR <br />did receive notice of the Duckworth Pit Application. <br />4. Gene Steve Hardison (Parcel # 131316000013) is noted on the "Names of Owners of <br />Property Within 200-feet" on Exhibit C. However, the parcel was not identified on the map <br />portion of Exhibit C. Please revise Exhibit C to include the location of the property owned <br />by Gene Steve Hazdison. <br />5. Water lines within 200 feet of the affected land were noted on Exhibit S, but were not <br />depicted on the maps. Please identify the location of the water lines and assure the Division <br />Office of Office of Colorado <br />Mined Land Reclamation Active and Inactive Mines Geological Survey <br />
