The decision date for your application is scheduled for <br />March 5, 2007. Therefore, please respond on or before 1:00 PM on March 5, 2007. (Your <br />response should include two (2) copies). Please be advised that your conversion application may <br />be deemed inadequate and maybe denied on Mazch 5, 2007 unless the remaining adequacy <br />review items listed below aze addressed to the satisfaction of the Division. (You may also elect <br />to waive your right to a decision within 90 days and submit a written request to extend the <br />decision date to allow more time for submittal of your written response. You must submit a <br />written request by no later than 4:00 PM on March 5, 2007 if you elect to pursue this option). <br />6.4.12 EXHIBIT L -Reclamation Costs <br />10. The Applicant's proposed reclamation cost estimate for each phase of the operation includes <br />a 20% cost for slurry wall installation. Please note that in order to qualify for the 20% option, the <br />operator will need to demonstrate the following requirements have been met prior to exposure of <br />groundwater at the Bokelman-Walters site: <br />• Submittal of a copy of the slurry wall installation QaQc procedures to be followed. <br />• Submittal of a final construction report with details of the slurry wall installation effort, <br />geologic conditions encountered, and any specific design modifications performed during <br />construction do to unforeseen field conditions. <br />• Certification from a registered professional engineer that the slurry wall has been installed <br />according to the design specifications and QaQc procedures, and that the slurry wall will <br />Office of Office of <br />Mined Land Reclamation Denver Grand Junction Durango Active and Inactive Mines <br />
