3. Naneomollanee Notifleacion
a) If, far any reason, the per alttee does not comply with or will
be unable to comply with aay discharge limicacioas or standards
specified in this permit, the permittee shall, ac a minimum,
provide the Aacer Quality Control Division and E?A with the
fallowing information:
(±) A deseripc±oa of the d±scharge and cause of noncompliance;
(ii) She period of noncompliance, Including enact daces and
times and/or the anticipated time when the discharge will
return to compliance; and
(111) Steps being taken to reduce, e1i=+nace, and prevent
recurrence of the noncomplying discharge.
b) She perm/ttee shall report the following Snstanees of
noncompliance orally wl[hin twenty-four (24) hours from the time
Che permittee becomes aware o the noncompliance, and shall mail
to the Div isioa a written report within _-°ive (5) days after
becoming aware of the naucamollaace:

(i) P.ay instance of noncompliance which may endanger health
ar Che environment;
(11) Any unanticipated bypass;
(iii) Any upset which causes as esceedance of any effluent
limitation is the permit;
(Sv) Aay shill which causes aay effluent limitation to be
violated;
(v) Daily aaximum vialacions for aay tozle pollutants or
hazardous subscaaces limited by ??3T_ I-A of G`:Ss permit
and specified as requiring 34 hour not*_fication.
e) The peraittee shall report all ocher instances of non-compliance
not requiring 24-hour notification at the time Discharge
Monitoring Reports are submitted. The reports shall contain the
information listed in sub-paragraph (a) of this section.
