'~ <br />Dear Mr. Austin: <br />The Division of Minerals and Geology has beguri the review process for the above <br />referenced 112c application. The application was considered complete on April 16th, <br />2004. Therefore, the Decision Date is currently set for July 15th, 2004. Please insure that <br />any and all correspondence concerning the review process of this application is reflected in <br />the copy of the application located at the Mesa County Clerk's Office. <br />A Preliminary Adequacy Review (PAR) has been accomplished. Please clarify or correct <br />the following noted issues: <br />1- The Division requires that roads that are significantly upgraded for the purpose of <br />access to a mining site be included within the permit boundaries. Text within the <br />proposed Mining Plan denotes major upgrades to the haul road- and a bridge from <br />Kannah Creek to the site. Therefore, the applicant must file an amendment to the <br />application addressing the addition of the haul road into the proposed permit <br />boundaries. Please refer to Rule 1.8.1(1).(2) and (3) concerning the amendment to <br />a 1 12c application. All maps and text must reflect the amended permit boundaries. <br />Also, the 200-foot requirement for man-made structures and notices to adjacent <br />landowners is applicable. <br />2) Rule 6.4.3 (e) requires a description of the type of vegetation covering the affected <br />lands. The description on the map of "brushy areas" is too vague, Please describe <br />the vegetation community in better terms on the map. <br />3) The supplied maps do not have the reauired signature of the qualified person who <br />prepared them as required by Rule 6.2.112-Ib). Please supply one original and one <br />copy of corrected versions of the maps with signature and any other issues noted in <br />the application review process. <br />Office of Office of .
