The signed voucher is being returned to you. <br />The applicant must choose one of the existing primary future land uses and <br />primary present land uses. "Creekbed" is not acceptable. Please see page three <br />from the enclosed 110 application package. I have enclosed a copy for <br />correction. <br />FXHTRiT F• <br />A Mining Plan Map and Reclamation Plan Map must be submitted. Please see page <br />ten, for instructions on map submittals. Maps should be drawn to a scale no <br />smaller than 1 inch 50 feet. <br />EXHIBIT F: <br />Please list other permits and licenses necessary for operation of this permit. <br />Example: Fugitive Dust Permit from the State of Colorado. <br />EXHIBIT I: <br />Proof of filing with the County Clerk was not submitted. The Division must <br />receive proof that the application was submitted to the Phillips County Clerk <br />and Recorder for public review. <br />
