<br />CDOT Right-of-Way:
<br />A. Written clearances for Threatened or findangcrsd Species,
<br />B. Wetlands Clcaranc0a.
<br />C. Ai~chaoologica] and Paleontological clearances. Any discoveries shall be mi gated in confnrmancc with
<br />section 107.23 of the standard specifications for road and bridge construction, l teat edition. A surface survey
<br />moat be performed and a written clearanco provided to the Departmatt and/or i uing authority prior to
<br />commencement of any construction.
<br />It is the responsibility of the Permittee to assure that no such sifts are presort p or to commencement of
<br />construction activities. All identified sites shall be mitigated to the satisfaction f the appropriate agency prior
<br />to proceeding with work activities.
<br />Wetland mitigation shall be completed in conformartoe with the policies, prase and regulations of the
<br />Army Corps of Engineers, Ftderal Highway Administration and the State of Co orado, as may be applicable.
<br />12. All costs associated with Ate installation of this aacass are the tesponatbility of a Permittee. This includes
<br /> the design, construction, utility relocations, testing of materials, and inspections
<br />13. Any work within CDOT Right-of-Way shall begin after 8:30 am and sll work d eggiptttent shell be off the
<br /> highway EHFORE 3:30 pm Monday thru Thumday and 12:00 Noon an Friday.
<br />14. No work within the roadway and the State Highway Right-of--Way is allowed at t, on Satwdays, Sundays,
<br /> on bolidays, during adverao weather conditions, or between November first and pril fifteenth of each year
<br /> without prior approval from the Department.

<br /> SUSPBNSION OF WORK BY ORDBR OP THE DFd'ARTMHNT 1NSPHGTO OR THE ISSUING

<br />16. The State requires a Certlflcate of Insurance prior to commencing any work on a highway right-of--way. Alf
<br /> vendors, contractors, and utility companies shall procure, at their own expense, maintain for the duration
<br /> o(the work period, the following minimum invtuattce coverage:
<br /> A. Standard workman's compensation and employct"s liability, including setup lions] disease, covering all
<br /> empioyoes engaged in performance of the work at the site, in the amounts requl d by State Statues.
<br /> Employer's Liability Insurance at a minimum of $500,000 each accident and $5 ,000 each disease,
<br /> B. Comprehensive General Liability at a minimum of 51,000,000 Hach Occu • $1,000,000 Personal
<br /> Injury; $2,006,000 Products/Completed Operations; $2,000,000 General Aggre te. CDOT will be endorsed
<br /> as an Additional Insured by the contractor and all subcontrectora. Completed O erationa coverage will be
<br /> provided far a minimum period of two years following the conditional aeceptan a of Work.
<br /> C. Commercial Automobile Liability will cover all owned, non-owned, and bi vehicles with a minimum of
<br /> 51,000,000 combined single limit bodily injury and property damage. The polio will protest CDOT es an
<br /> Additional Insured and be written on an Occurrence farm.
<br /> D. Professional Liability Insurance, The contractor will ensure that all subeon etors procure and maintain a
<br /> minimum of 51,000,000 Bach Ocouronce and $1,000,000 Armual Aggregate.
<br /> E. Umbrella or Excess Inability at a minimum of $1,D00,000. The policy wiU c main a clause stating that it
<br /> takes effect (drops down) in the went the primary limits art impaired or exbaus .The policy will be written
<br /> on an Occurrence form.
