Comprehensive inpeuion - <br />Additional STi'MP Reouirements for Sites Seeking Permit Terminations In the evem that the permtttee <br />wishes to remediate an inacive mine site and terminate permit coverage, prior to remediarion activities, the <br />SWMP must be amended ro incltuie the following items, and the permittee must notify the Division of the <br />change in status. <br />e. Additionallrems <br />1) Doutmemation of land owner permission (if the perntittee is not the owner) <br />2) Photographs <br />3) Description of remedial action to be performed <br />4) Ducription of BMPs during remediation auivitiu <br />5) Procedure for disposal of any water stored within mint waste <br />6) Descriprion of permanem BMPs to be used, Etna! site stabilization, and ultimate land use <br />3. SWMP Development. Coordination and Format <br />Some auivities required under the SWMP may already be in place. However, the SWMP will require the <br />permittee to coordinate these auivitiu with any necessary new aaivitiu in an orderly manner, so that the <br />result is the reduuion or elimination of pollutants reaching state waters from areas no; limited by numeric <br />• effluem limitation. <br />It is the permitree's responibiliry to notify the Colorado Division of Minerals and Geology (CDMG) of any <br />changes at their site resulting from the implememation of the SWMP. This norifiwtion allows the CDMG to <br />review the SWMP and incorporate any potential revision which might be needed imp the facility's <br />reclamation permit. It is important to verify that the site reclamation permit matches the SWMP. -Such <br />information is usually contained in Exhibit G of 110 reclamation permits. <br />The SWMP may rely upon information developed for other similar pollmion control programs such as the <br />SPCC (Spill Prevemion Comrol and Coumermeasuru) plan, Vofumary Cleanup Plan, or the Mined Land <br />Reclamation Plan. Appropriate portion of these other plans may be incorporated in the SWMP by reference, <br />as long as a complete SWMP can be reproduced and submitted to the requuting agency within the time frame <br />specified in the request. Those portion of the referenced documems must be incbrded in the submittal. The <br />SWMP must also be readily available ro an on-sire inpeuor. <br />C. Comprehensive Facility Inspections <br />For active mining operations, the permittee shall make a thorough inspeuion of their stormwater management <br />rystem, at least twice per year (n the spring and fall), as detailed in Part l.D.S of the permit. These inpeuioru <br />must be documemed and summarized in the Annual Report {see Part I.E.1 of the permit). <br />For inactive mining operation, a thorough inspection of the stormwater managemem rystem is required at Ieast <br />once per year. Where annual inpeuion of inauive mine are imprauicable, an inspeuion and certification of <br />compliance are required once every three years by a Registered Professional Engineer, as detailed in Part I.D.S of <br />the permit. Thee inpections must be documemed and summarized in the Compliance Report (see Part l.E.1 of the <br />permit). <br />For remediation projects, the permittee shall mdse a thorough inspeuion of their srormwmer rttanagemem rystem ar <br />Least once per year (during the field season). Where annual inpection of inauive mines are imprauicable, an <br />inspection and certification of compliance are required once every two years. These inpection must be <br />documented and summarized in the Annual Report (see Part I.E.1 of the permit). <br />
