The Discharge Monitoring Repoli forms shall be filled out <br />accurately and completely in accordance with requiremeata of this permit and the imtructiom oa the forma. They shall <br />be signed by as authorised person as identified in Put I.D. <br />3. Acute WEI' Testing-Outfalls 004. 023. 024 and 026 <br />(a) Testing and Reporting Reauiroments <br />Testa shall be done at the finqueacy listed in PaR I.B.1. Teat results shall be reported along with the Discharge <br />Monitoring Repoli (DMR) submitted for the reporting period during which rho sample was takes. t<.e., WET testing <br />rrsulta for the first calendar quarter ending March 31 shall be reported with the DMR due Apci128.) The results <br />shall be submitted on the Acute Toxicity Teet report form, available from the Division. Acute testa will 6e replacement <br />static testa of a single effluent grab sample. <br />(b) Failure of Test and Division Notification <br />M acute WEI test is failed whenever the LCp, which represents an ~:^~^« of the effluent conomtrstioa which is <br />lethal to 50% of the test org..~ in the time period prescribed by the test, is found to be less then or equal to <br />100% effluent. The permittee must provide written notification of the failure of a WET test to the Division, along <br />with a statement as to whether the Preliminary Toxicity Luideot ('PTI')/Toxicity Identification Evaluation ('TIE") <br />investigation or accelerated testing is being performed (see mxt section). Notification mtut be received by the <br />Division within 14 calendar days of the demonslratios of acute WET in the routine required test. <br />'Demonstratiaa• for the purposes of PaRa I.B.4tb),(c),(d) and (f) mean no later thaw the last day of the laboratory <br />test. <br />(c) Automatic Compliance Schedule Uoon Failuro of Test <br />If a routine acute WET test is failed, the following auWmatic compliance schedule shall apply. As part of this the <br />permittee shall either. <br />(i) Proceed to conduct the PTllITE investigation as described in PaR I.B.4.(d), or <br />(ii) conduct accelerated testing ming the single species found to be more sensitive. <br />• TF accelerated testing is being performed, the permittce shall provide written sotiTcatios of the rtsdfs within <br />14 calendar days oC completion of the "Patters of Toxiaty"/"No Toxidty" demomtratioa Testing will be at <br />lean once every two weeks for up to five tests until 1) two coasxutive tests fail or three of five tests fail, in which <br />case a pattern of toxicity has bees demoasuated or, 2) two consecutive testa pace or throe of five test pass, is <br />which case no pattero of toxicity has bees found. <br />
