The forma shall be mailed to the agencies listed below so they are received no later than the 28th day of the <br />following month. If no discharge occurs during the reporting period, 'No Discharge' shall be reported. <br />The DMR forms consist of four pages -the top 'original' coPY~ sad three attached no-carbon-required copies.
