4. Aggregate Industries must agree to include in its application, for a substitute water supply <br />plan, all probable mounding of water predicted in the groundwater model. <br />5. Aggregate Industries must add to Exhibit G Tetra Tech's recommendation that water <br />levels in the monitoring wells be measured monthly until extraction begins and quarterly <br />unfil reclamation is complete and the DMG releases the financial warranty or until the <br />Mined Land Reclamation Board releases A. I. from further monitoring responsibility. <br />Aggregate Industries must agree to provide the results of the monitoring annually with <br />the permit annual report. <br />6. Aggregate Industries must affirmatively state that baseline water levels have been <br />established for all wells within 600 feet of the proposed affected land. <br />7. Aggregate Industries must agree to mitigate any significant negative impacts, caused by <br />dewatering operations, to the water supply for wells of owners within 600 feet of the <br />proposed affected land. <br />Exhibit L -Reclamation Costs <br />1. Exhibit L will be reviewed after Aggregate Industries or its representative responds to this <br />adequacy review. <br />
