During the construction of this access and shouldering widening operations, the <br />Pcrmittee is responsible for any damage done to Lhe 'sighway or any highway <br />faci]ities and ail expenses incurred for repair. All repair will be done prior <br />to continuing any construction. <br />11. Should the use of this access create a traffic hazard, as determined by th@ <br />Department, appropriate auxiliary lane(s) (acceleration, deceleration, and <br />left-turn lanes) shall be installed and constructed to Colorado Department of <br />Transportation standards and specifications at no cost to the Department. <br />12. Installation of auxiliary lanes may be required, as determined by the <br />Department, if the access volumes meet or exceed the design requirements in <br />Section Four of the State Flighway Access Code (2 CCR 607.-1, as amended}. <br />Design and construction of. warranted lanes shall be completed at no cost to the <br />Department. <br />13. Final acceptance will be by A. A. Shablo, Rcgi.on Transportation Director and <br />Jon T. Incorporated as part of this permiC aze the following: Application for Access <br />Permit (Form No. 137) including attachments, both sides Of permiC Pdge 1, <br />Permit Pages 2, 3 and ~, Exh.ibit A, and Colorado Department of Transportation <br />M65 Standard M-2D3-]. <br />16. This permit. is issued in accordance with the State Highway Access Code and is <br />based upon the information submitted by the permittee. Changes in traffic <br />volumes, drainage, type ct traffic or other operational aspects may render this <br />permit void, requiring that a new application be submitted f.or review based <br />upon existing and anticipated future Conditions. <br />If necessary, minor changes and additions to this permit shall be ordered by <br />the Maintenance Foreman to meet unanticipated site conditions. <br />All required access improvements shall be installed prior. to the herein <br />authorized use of. this access. <br />]'/, The need for a side drain culvert will he determined at the time of <br />construction by the Maintenance Foreman. 7f required, a new 18 inch corrugated <br />steel or 21 inch x 15 inch arched steel pipe shall be used, as directed by the <br />Maintenance Foreman. 'f he Maintenance Foreman may also require an increase in <br />the size of the side drain t.o adequately handle the historical drainage flows. <br />The side drain culvert sha)]. be of sufficient ]ength to extend a minimum of two <br />feet. beyond the toe of the access slope. The side drain shall be installed in <br />a manner which provides proper drainage, with no less than twelve (12) inches <br />of compacted backfill material over the top of the culvert. In accomplishing <br />this requirement it may be necessary for the permittee to perform some minor <br />drainage ditch shaping. The side drain shall NOT be i.nstallad prior~to review <br />and authorization from the Maintenance Foreman^. <br />18. No drainage from this site shall enter onto the roadway of the highway, <br />
