This permit is issued to permit and allow the construction of a new commercial <br />access to State Highway 145, located at approximate milepost 63.226 left. <br />2, The access shall serve that indicated on Page one Ill. ~ , <br />3. Use of this direct highway access for the grave]. pit operations shall continue <br />until such time that an alternate subdivision road or general street system <br />becomes available. The Department may require the Permittee to close and <br />remove this access whon an alternate roadway system iS established. <br />4. you must Contact Dave Reece at the Department Shop located On State Highway 62 <br />in Ridgway, Colorado or telephone (3031 626-5588 (if there is no answer, ca 11 <br />385-1650 and leave a message} at least two 12) working days prior to beginning <br />~anyy access improvements or construction within the highway right-of-way. This <br />includes but is not limited to the installation of side drain culverts and <br />fencing. Failure to comply with this condition may result in revocation Of <br />this permit. <br />5. A construction traffic control plan must be submitted to the Region 5 Traffic <br />Engineer (3803 N, Main Ave., Suite 300, Durango, Co. 81301, or telephone (303) <br />385-]400) for inspection and approval, a minimum of five (5) working days prior <br />Lo commencement of any access construction or shoulder widening. The traf.f.ic <br />control plan shall be in conformance with the Manual on Uniform Traffic Control <br />Devices, and ell other applicable standards. No lane closures or traffrc <br />detours of eny'nature will be allowed unless pre-approved by the Department. <br />6, Clearing and grubbing within the highway right-of-way sha]1 be completed in <br />conformance with Section 20] of the Colorado Standard Specification for Road <br />and Bridge Construct•i.or. (1991 Editionl• Al.l cleared marketable timber within <br />the highway right-of-way shall be cut into four (4) foot sections, and be <br />Clocked and stacked on the highway right-of-way for the public's use. Stacking <br />within the highway right-of-way shall not create a safety hazard, and shall be <br />as directed by the Maintenance Foreman. <br />7. At. the direction of the Maintenance Superintendent, the Permittee shall <br />excavate, haul and place, as shoulder widening, all sul.table embankment <br />material removed within the existing highway right-of-way during the <br />construction of this access and shoulder widoninq operations. <br />B. 1'he Permittee is responsible for keeping the State Highway 195 highway trave] <br />lanes clear of any mud or debris tracked onto it during the construction of <br />this access and the shoulder widening operations. Any mud or debris which is <br />tracked on to the highway travel lanes shall be removed daily or as otherwise <br />directed by the Maintenance Foreman. <br />9. The Permittee is responsible for any utilities disrupted by the construction of <br />this access and all expenses incurred for repair. <br />
