Long <br />RE: Washington County Road & Bridge De pt., File No. M-2001-012 Division Director <br />Shook Gravel Pit; Adequacy Review <br />Dear Mr. Kuntz, <br />Listed below are adequacy review comments on the Shook Gravel Pit New 112 Application, File No. M- <br />2001-012. The Division of Minerals and Geology (Division) is required to make an approval or denial <br />decision no later than May 30, 2001, therefore, your response to the following adequacy review concerns <br />should be submitted to the Division as soon as possible. <br />1. Please submit the proof of notice to all owners of record of surface lands, mineral owners, easement <br />holders, and owners of record of surface lands that are within 200 feet of the boundary of the affected <br />land. Proof of notice may be return receipts of a Certified Mailing or by proof of personal service. <br />On April 18, 2001, the Division received proofof mailing for only [he mineral owners. Please submit <br />the remaining proof of notices for owners of the surface land, easement holders and surface <br />landowners within 200 feet of the affected land. <br />Rule 6.4.3 Exhibit C- Pre-minine and Mining Plan Maps <br />2. Map Exhibit C does not label the existing topography of the area with contour intervals to portray the <br />direction and rate of slope of the affected land. Please revise the map to depict the existing <br />topography of the area and direction of flow of Brunker Creek. <br />3. Please indicate the acreage for the permit area versus the acreage of the affected land. Affected land <br />includes surface disturbance such as roads, land excavations, ponds, storage areas, stockpile piles and <br />working areas. <br />4. Rule 6.4.3(g) requires the applicant to show the owner's name for all structures within 200 feet of the <br />affected land. Please indicate the owner's of all structures on the permit area and within 200 feet of <br />the affected land. This includes homes, roads, wells, fences, utility poles, gas lines, etc... <br />Rule 6.4.4 Exhibit D- Minint: Plan <br />~. Please indicate the equipment used to remove the topsoil and mine the pit. Please also indicate if a <br />working and storage facility will be constructed. Such facility will need to be depicted on one of the <br />Exhibits. <br />The applicant depicts a number of topsoil piles inside the permit area. Please inform the Division <br />how such piles will be protected from wind and water erosion. The applicant should specify a <br />temporary seed mixture, timing of such seeding, the seed rate and application method. <br />7. Please state if any overburden material will be salvaged and stockpiled for use during reclamation. <br />Please indicate where overburden material will be stockpiled on the Exhibit. <br />
