Attach two copies of an accurate map of the area affected which map <br />shall include, but not be limited to the following: <br />a. The map shall be made and certified to by a registered land <br />surveyor, professional engineer registered in the State of <br />Colorado, or a qualified person which shall show the date on <br />which the map was prepared and the north point. <br />b. Identify the area on the map with colored ink or some other <br />manner which corresponds with the data sheet. <br />c. Indicate adjacent deep mining and adjacent surface owners. <br />d. Indicate the name and location of all streams, creeks, or other <br />bodies of water, roads, buildings, cemeteries, oil and gas <br />wells and lines, and power and communication lines on the area of <br />affected land and within two hundred (200) feet of all boundaries <br />of such area. <br />e. Indicate the total area to be involved in the operation including <br />the area to be mined and the area of Land affected by surface <br />waste or refuse dump roads, buildings, stockpiling of the subsoil <br />and topsoil. <br />f. Show the location of test boring holes. <br />g. Indicate the topography of the area with contour lines of <br />sufficient detail to portray the direction and rate of the <br />slope in question. <br />-3- <br /> <br /> <br />
