Suspended or Revoked Permits <br />• <br />HGTI nor any companies in the Organizational Family Tree (Attachment 3-2) <br />have not had any State or Federal mining permits suspended or revoked <br />during the past five years. <br />List of Violations <br />HGTZ has not received any notices or violations in connection with any <br />surface coal mining operations during the past three years HGTI nor any <br />other companies in the Organizational Family Tree (Attachment 3-2) do not <br />have any unabated cessation orders or violation notices. <br />DMG approved the Succession of Operators application for the Hayden Gulch <br />Mine, Permit No. C-80-003, which transferred Permit No. C-80-003 from 8-G <br />Coal Company to HGTI oa January 21, 2003. The Hayden Gulch Miae, while <br />operated and owned by H-G Coal Company was issued a notice of violntion on <br />October 10, 2000. The notice of violation, CV-2000-008, was issued by DMO <br />because H-G Coal Company failed to submit to DMG a certificate certifying <br />• that the permittee (H-6 Coal Company) had public liability insurance in <br />force, and failed to make the certificate available to the public et a <br />location in the area of mining. <br />DMG terminated notice o£ violation CV-2000-008 on October 13, 2000 aftez H- <br />G Coal Company abated the violation by providing the required certification <br />of insurance.
