It is not clear if a reclamation map was provided in the application packet. Please provide <br />a reclamation map or specify which map is reclamation map. If map #5 is the reclamation <br />map, please include the following information on the map. <br />2. Please show which roads if any will remain on site <br />3. Please show the gradient of all reclaimed slopes (horizontal vertical) sufficient to describe <br />the post mine topography;. <br />4. Please include the proposed mine boundary in relation to the items requested above. <br />6.3.6 EXHIBIT F -List of Other Permits and Licenses Required <br />1. Is a county conditional use permit required to mine in Park County? No county permit was <br />noted in this exhibit. <br />6.3.7 EXHIBIT G -Source of Legal Right-to-Enter <br />1. On most Federal land mine operation two items need to be provided as a source of legal <br />right to enter, please provide both of the required documents. <br />a. The BLM Load Claim, including the CMC number <br />b. An approved Forest Service Plan of Operation <br />2. U.S. Forest Service permit: The Draft Plan of Operation was submitted along with the <br />Division applicant packet. The Division reviewed the Draft Plan of Operation attachment <br />for consistency with the Division permit application, the two documents were found to be <br />consistent with each other (Memorandum of Understanding between the USFS and <br />Division 5/8/2004).
