Cottony <br />t// Division Director <br />Natural Resource Trustee <br />M-2003-073, New 112c Permit Application Preliminary <br />DMG has completed the preliminary adequacy review of your application fora 112c Permit for the Juniper <br />Mountain Gravel site. The application cannot be approved until the following issues are adequately <br />addressed. Please submit a response to the following issues as soon as possible, but no later than <br />December 3, 2003 so the decision due date set for December 15, 2003 can be met. If you cannot respond by <br />December 3, 2003 please submit a written request for extension of the decision date. If your response, or an <br />extension request, are not received by December 3, 2003 this application may be denied. <br />ADEQUACY ISSUES <br />Exhibit C -Pre-minino and Mining Plan Maos <br />1. Please submit the information required by Rule 6.4.3(a), (e), and (g). <br />2. Please indicate where topsoil and overburden stockpiles will be located as the operation <br />approaches the final eastern mining boundary. The current stockpile area indicated should be <br />reclaimed at that point in time. <br />3. All maps, except the location maps, need to meet the requirements of Rule 6.2.1(2). <br />.Exhibit D - Minino Plan <br />4. This application was originally submitted as a conversion of your 110c permit. Because this is <br />now a new 112c application, all information required by Rule 6.4.4 must now be included in this <br />application, References to information in Permit No. M-2003-011 are not acceptable. Review of <br />the 111 applications submitted by Lafarge for the portion of the site that approximates the <br />western half of this proposed area, may provide some of the information that needs to be <br />included in this application. The rule citations will be different in those 111 permits. <br />5. Please specify how overburden will be selectively handled and separated from the topsoil to <br />protect the topsoil resource. <br />6. Please specify all types and amounts of hazardous materials to be used and stored on site and <br />submit design specifications for spill containment structures for all containers of hazardous <br />materials to be stored.- Additionally, please incorporate spill reporting language into the mine plan <br />Office of - - Office of Colorado <br />Mined Land Reclamation Attive and Inactive Mines Geological Survey <br />
