ADDENDUM 3 <br />• Sedimentation Pond 006 - Phase 11 <br />As-Built Certification <br />Introduc Tian <br />The embankment and spillway structures of Sedimentation Pond 006 - Phase I was constructed <br />in the foil of 1989 and according to the designs contained in the approved permit. The <br />as-built conditions of these structures are shown in Exhibit 13-6. This certification <br />covers the Sedimentation Pond 006 - Phase 11. <br />Construction Summary <br />In accordance with Rule 4, Part 4.05.6(7) of the Rules and Regulations of the Colorado <br />Mined Land Reclamation Board pursuant to the Colorado Surface Coal Mining Reclamation Act <br />34-33-101 et seq., 1, Ronald J. Gehrk e, hereby certify that the construction of <br />Sedimentation Pond 006 - Phase Il was inspec [ed end constructed under my supervision. <br />• From September 20 until September 27, 7989 during Phase I construction, I was on site to <br />inspect the construction of this structure. The construction of Phase I was supervised on <br />site by Rey Armijo, a graduate civil engineer, and Chuck McCUlloh, Operations Engineering <br />Manager a[ the Seneca II-N Hine. Rey and Chuck assured all work was performed in <br />accordance wish the design and the specifications. 1 inspected the Phase I structure on <br />November 7, 1989, May 22, )990, June 18-20, 1990, end again on July 27, 1990 to verify the <br />ponds conditions. I also inspected the completed Phase I structure on September 5, 1990 <br />and again on November 79-20, 7990 to verify the es-built conditions. 1 inspected the <br />Phase 1[ construction on August 1, 1991 end again on November 21, 1991 to verify the <br />as-built conditions. I, Ronald J. Gehrke, em a 1977 graduate in civil engineering end 1 <br />have been a Registered Professional Engineer in Colorado since November 22, 1982. <br />The purpose of my inspection and certification was to ensure proper construction of <br />' Sedimentation Pond 006 - Phase 11 end to satisfy the requirements of Rule 4.05.6(7) of the <br />Colorado Mined Land Reclamation Division. The materiel was placed in the embankment to <br />meet or exceed the design specification (see Addendum 4 for compaction information). This <br />as-built report confirms the construction of this structure meets or exceeds the Rules and <br />• <br />
