<br />Item 9 -The applicant signs under item 9, if they are applying for the first <br />time under the general permit program or if the permlttee is applying <br />for re-certification under the general permit program but has had <br />operational changes at the facility. <br />1) The application form shall be signed as follows: <br />(a) In the case of corporations, by a principal executive <br />officer of at least the level of vice-president or his or <br />her duly authorized representative, if such representative <br />is responsible for the overall operation of the facility <br />from which the discharge described in the application <br />originates. <br />(b) Ia the case of a proprietorship, by a general partner. <br />(c) Ia the case of a sole proprietorship, by the proprietor. <br />(d) In the case of municipal, state, or other public facility, <br />by either a principal executive officer, ranking elected <br />official, or duly authorized employee. <br />2) Both the owner(s) and operator(s) shall sign the form. <br />-2- <br />
