<br />STATE OF COLORADO <br />GENERAL PERMIT APPLICATION RENEWAL ONLY FOR <br />CONSTRUCTION AGGREGATE WASTEWATER DISCHARGE APPLICATION INSTRUCTIONS <br />This application is for use by all industrial dischargers in the construction <br />aggregate (sand and gravel) category. It is suggested that the applicant <br />contact the Mined Land Reclamation Division of the Colorado Department of <br />Natural Resources concerning reclamation rules and regulations. <br />All items of the application moat be completed accurately and in their entirety <br />or the application will be deemed incomplete and processing of the permit x111 <br />not begin until all information is received. Applications shall be submitted <br />by certified mail or hand delivered, only to: <br />Colorado Department of Health <br />Water Quality Control Division <br />Permits and Enforcement Section <br />4210 East 11th Avenue, Room 200 <br />Denver, Colorado 8C220 <br />Item 1 -All owners of the property must be identified. <br />Item 2, 3 sad 4 - Self explanatory. <br />Item 5 -Please review the copy of the certification sent to you. Please <br />make changes or additions xhere necessary. Include supporting data <br />as needed. Please review the copy of the certification to determine <br />Sf any chemicals are being used which xere not originally discussed <br />in the certification. For all new chemicals, please include the <br />Materials Safety Data Sheet (MSDS). <br />Item 6 and 7 - These questions deal xith atormwater discharges xhich are not <br />presently covered under this general permit. When necessary include <br />a map. <br />Item 8 -Include only data currently existing. It is not necessary to <br />collect new data. <br />-1- <br />
