1. The address where the emergency fias occurred. If the address <br />is given as a rural route, box number of general area, obtain <br />additional information to further identify the location. <br />2. The name of the caller. <br />3. The telephone number of the caller. <br />4. Personal estimate of the information from the caller as to the <br />severity of the situation. <br />5. Type; of structures or areas involved; i. e., school buildings, <br />public assembly areas, critical area locations, etc. <br />6. Action that has already been taken by persons at the emergency <br />site. <br />7. An estimate of how long the problem has existed. <br />8. The traffic situation in the area involved. <br />9. Other persons or agencies that have been notified; i.e., fire <br />• department, police department, ambulance service, maintenance <br />personnel. <br />10. Any other information that might be helpful. <br />11. Time of the call and the date. <br />The person receiving the call may also give instructions to the <br />caller, depending upon the situation. If the emergency involves gas in <br />a building, then if deemed necessary, instruct the caller to clear the <br />building or the area involved and caution about operating electrical <br />switches, smoking, open flames and operating equipment. <br />After obtaining the above information, the appropriate personnel <br />shall be notified so that emergency action will be taken immediately. <br />• -6- <br />
