22. Please ensure that the Reclamation Map contains sufficient information to show the direction and rate of slope,
and that all features which are not to remain after mining are removed, as required by this rule.
23. [f there are to be other underground openings, please show their expected, approximate locations. Please be
reminded that all areas to be affected must be within the permit boundary.
Exhibit F - Oiher Permits (Rule 6.3.6)
24. Please provide any discharge permit information you obtain, since this operation will likely have a perennial
discharge.
25. If lands managed by BLM will be affected by activities or discharges, please provide copy of any permit or
agreement which allows you to affect BLM land.
Exhibit G - Ri¢ht to Enter (Rule 6.3.7)
26. This appears adequate for now, but please do not allow the agreement to expire, which it might do in 2004. At
this time, no further response is needed.
Other Items
27. Please provide a notarized affidavit from the publisher after the lagal notice has run.
28. Since the BLM is the adjoining landowner, please provide evidence of having mailed a notice to them
29. As previous materials from the Division have stated, all additional materials you provide to clarify or address
items in our adequacy review letters, must be also filed with the county clerk and recorder. Please provide receipts
for the additional filings of all such additional materials.
1 will contact you and Mr. Charles to schedule an onsite inspection, probably to occur later in August or early
September. I am co~dent that an inspection will help clarify some of the questions pertaining to the application
for this site.
The decision date is currently set for August 29, 2001. Please be reminded that if the questions above are not
adequately addressed by the close of business on the decision date, the application may be denied. If additional
time is needed for preparing or submitting your responses, an extension may be requested. Please make such
request in writing, either by mail or fax.
Many of the responses you provide will help us to determine a reasonable bond amount for this operation. Please
be as complete as you can.
If you have questions or comments, I may be reached at the Division's Durango field office: 701 Camino del Rio,
Room 125, Durango, CO 81301; tel 970/247-5193 or fax 970/247-5104.
Sincerely,
Bob Oswald
Environmental Protection Specialist
cc: Les Charles, Mine Superintendent, Midas Mining Co., Inc., 710 N. 11th St., Gunnison, CO 81230
Julie Coleman-Pike, BLM, 216 N.
