<br />APPLICATION E)CHIBITS A-T ~ <br />r <br />two copies of Application Exhibits A-F muat be attached to the Application Forn upon <br />submittal to the Division. Each Application Exhibit must be presented separate from the <br />other exhibits. Please type the title of each exhibit at the top of each Application <br />Bxhibit page. Please number the pages of Application Exhibits A-P consecutively for <br />ease of reference. The following sections describe the minimum infotznation that :rust be <br />supplied for each Application Exhibit. <br />APPLICATIO!! E~iIHI~f A - Location t?aa <br />Section 35-]2-110(1)(a)(E) requires that you supply the Division wits a brief description <br />and map of the location of the proposed mining operation relative to streams, drainages, <br />or other prominent Features and man-made structures. This map is not the same as the <br />Affected Lands !!ap to be supplied as Application Exhibit C. The Division requires that <br />the Applicant/Operator use a standard 1:24,000 scale topographic map produced by the <br />U.S. Geological Survey. <br />APPLICATION EAiIHIt B - Leeal Description <br />The legal description must identify all affected lands, specify the acreage of the <br />affected lands, and be adequate for field location of the property. It should be a <br />description by reference to township, range, and section, to at least the nearest <br />quarter-quarter section. A metes and bounds survey description is also acceptable. <br />ifiere applicable, the street address or lot number(s) may be used. This inforration is <br />available from the County Assessor's office or maps Erom the U.S. Geological Surrey. <br />APPLICATION E'PyIHIT C - Affected Lands Nao <br />Section 74-32-110(1)(a)(V) of the lSined Land Reclamation Act requires that the Applicant/ <br />Operator provide the Division with a map sufficient to determine the location of the <br />affected land and existing and proposed roads or access routes to be used in connection <br />with the mining operation. this map is not the same as the Location hap supplied as <br />Application Exhibit A. This map must be drawn to a specified scale, and identify the <br />location and type of activity which will occur in each portion of the affected lands. <br />this map must identify the size and location of any underground openings, waste piles, <br />topsoil stockpiles, ditches, ponds, roads, mine benches, and other facilities which will <br />be developed as part of the mining or reclamation operations. This map must also ident- <br />ify any pernanent man-made structures within two hundred Eeet of the affected area, <br />including houses, barns, fences, utility corridors (above or below ground), irrigation <br />ditches, roads, bridges, wells, pipelines, water storage facilities, railroad tracks, <br />cemeteries, communication antennas, etc. <br />8276v -8- <br />
