B. STORMWATER MANAGEMENT PLAN -ACTIVE MINES (cont.)
The plea shall identify potential sources of pollution (including sediment) which may reasonably be expected to affect the qualiry
of atormwater discharges associated with the mining operation. In addition, the plea shaft describe and ersure the
implementation of best management practices (BMPs) which will be used to reduce the pollutants in stormwater discharges
associated with miamg activity.
As a condition of this permit, alive mining operations must certify the completion and implementation of the SWMP required
under this part. At~ve minittg optrations must ®plement the provisions of thew SWMP su a condition of this permit.
The Division reserves the right [o request and review the plans, and [o require additional measures to prevent and control
pollution, as needed.
The SWMP shall include the following items, at a m;n;mnm;
Description of Minute Activities
The plan shall provide a narrative description of the mining and associated activities taking place at the site which affect
ar may affect sWrmwater runoff intended to be coverod by this permit. The narrative description shall report the total
acreage within the mine site and an Kfi~+are of the number of acres of disturbed area. A general description of the
location of the mining site relative ro major transportation routes and communities shall also be provided.
2. Arta Subject W Effluent Lvnitations Guidelines
Each plan shall describe the portion of the mining site which contribute runoff to areas subject to effluent limitations
guidelines contained in 40 CFR subchapter N.

Each plan shall provide a site map or maps which indicate at a minimum:
- mininglmilliag site bomdaries and access and haul roads;
- equipment storage, fueling and maintemace arras;
- materials handling areas;
- arras used for storage of overburden, materials, soils or wastes;
- areas used for outdoor manufacturing. storage or disposal of materials;
- location of mine drainage or any otiter process water,
- tailings piles/pamds, both new and pre-existing:
- mine drainage or nay other process water discharge points; .
- springs, streams, wetlands sod other surface waters: and
- boundary of area that contributrss runoff to outfalls that are subject to effluent limitations guidelines.

4. Desaioton of Potential Pollutant Sounees/Material Inventor
The plan shall provide a description of potential sources which may reasonably be expected to add pollutants to
• stormwater discharges. The plan shall idendfy all activities and materials which may potentially be pollutant sources.
This description of soutoes should agree with sources identified on the map. The plea shall include, at a minimum:
a. An inventory and narrative description of significant materials (sx definitions in Part I.D. of this permit) that have
