Reporting of Daca <br />Reporting of Che data gathered.in compliance with Part I.B.1 shall be on a Quarterl~~ <br />basis. <br />Monitoring results shall be summarized for each calendar quarter and reported on <br />Division approved discharge monitoring report (DMR) forms, postmarked no lacer than c:.. <br />28th day of the month following the end of the quarter (for example, the Dt4t for the <br />first calendar quarter must be postmarked by April 28th). If no discharge occurs <br />during the reporting period, "No Discharge" shall be reported. <br />The first ori¢inal signed copy of each discharge monitoring report (DMR) shall be <br />submitted to the Division at the following address: <br />Colorado Department of Public Mealch and Environment <br />WQCD-PE-82 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be <br />submitted to the following agency: • <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8LTti-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permittee records. The Discharge Monitoring <br />Report forms shall be filled out accurately and completely in accordance with <br />requirements of this permit and the instructions on the forms. They shall be signed 'c <br />an authorized person as identified in Part I.D. <br />3. Storm Exemption - Discharge points 007, 009, 010, 012 and 013 <br />If the permittee intends to use the 10-year, 24-hour storm exemption, a report must ba <br />submitted within 90 days aftez the effective date of this permit, documenting that chz <br />facility is designed, constructed and will be maintained to contain or treat: <br />(a) The maximum volume of wastewater which would be generated by the facility durir.; <br />a 24-hour period without an increase In volume from precipitation; and <br />(b) The maximum volume of wastewater resulting from a 10-year, 24-hour precipitacic:. <br />event, including the volume which would result from all areas contributing <br />runoff to the individual treatment facility (i.e., all runoff not diverted fro:-. <br />the active mining area or the mill area). <br />Amendment 1 Issued 03/10/93 Effective 05/01/93 <br />Amendment 3 Issued •l`r~=%'~ Effective ;,~,~ ?V <br />
