~ MONITORING REQUIREMENTS

2. Reporting of Daca
Reporting of Che data gathered.in compliance with Part I.B.1 shall be on a Quarterl~~
basis.
Monitoring results shall be summarized for each calendar quarter and reported on
Division approved discharge monitoring report (DMR) forms, postmarked no lacer than c:..
28th day of the month following the end of the quarter (for example, the Dt4t for the
first calendar quarter must be postmarked by April 28th). If no discharge occurs
during the reporting period, "No Discharge" shall be reported.
The first ori¢inal signed copy of each discharge monitoring report (DMR) shall be
submitted to the Division at the following address:

The first duplicate signed copy of each discharge monitoring report (DMR) shall be
submitted to the following agency: •

The third and fourth copies are for the permittee records. The Discharge Monitoring
Report forms shall be filled out accurately and completely in accordance with
requirements of this permit and the instructions on the forms. They shall be signed 'c
an authorized person as identified in Part I.D.
3. Storm Exemption - Discharge points 007, 009, 010, 012 and 013
If the permittee intends to use the 10-year, 24-hour storm exemption, a report must ba
submitted within 90 days aftez the effective date of this permit, documenting that chz
facility is designed, constructed and will be maintained to contain or treat:
(a) The maximum volume of wastewater which would be generated by the facility durir.;
a 24-hour period without an increase In volume from precipitation; and
(b) The maximum volume of wastewater resulting from a 10-year, 24-hour precipitacic:.
event, including the volume which would result from all areas contributing
runoff to the individual treatment facility (i.e., all runoff not diverted fro:-.
the active mining area or the mill area).
