All such <br />insurance shall be written on a Comprehensive Form of <br />Policy. <br />In the event any of the hazards or exposures, normally <br />listed in standard policies as "exclusions", are involved or <br />required pursuant to granting the temporary use, then such <br />hazards or exposures shall be covered and protection <br />afforded under the policy and such exclusions must be <br />removed from the policy. <br />30. Annual Reports on Operations will continue to be due on or <br />before July 1 of each operating year, the operator shall <br />submit four (4) copies of a written progress report to <br />the Board of Adjustment. Said report shall include but <br />shall not be limited to the following: <br />a. Records and reports on blasting including, but not <br />limited to, a report identifyiny days of operation and <br />identifying all days on which the detonation of blasting <br />material within 25U feet of the ground surface has <br />occurred. <br />b, Explanation of reclamation efforts and progress made <br />with ceclaination including, but not limited to, a <br />pro~~rE•ss rep~~rt on reclamation identifying areas beiny <br />
