Lonh <br />Division Direnor <br />Your application has been reviewed to determine whether it is <br />adequate under the requirements of the Act and Regulations. Due to <br />your other permitted operation, M-77-076 - Green Bros. Gravel Pit, <br />the Division has revised the site name of your present application, <br />M-95-030, to Green Bros. Gravel Pit No. 2. Please use this <br />designation in all future references to M-95-030. <br />Several items have been identified that need to be addressed before <br />the application will satisfy the applicable requirements. Please <br />provide responses to each item as soon as possible. The following <br />items need to be addressed: <br />(1) The maps supplied in Exhibit E do not have the authorized <br />signature as required by Rule 6.2.1 (2)(b) in the Mineral <br />Rules and Regulations. Please obtain the authorized <br />signature. <br />(2) The application mentions existing retaining berms or dikes <br />on three sides and the Gill-Edwards drainage ditch. <br />However, only one of the berms (on the east side) is <br />identified on the maps. Please identify the location and <br />approximate dimensions of the other two berms and the ditch <br />on the maps. <br />(3) The estimated reclamation cost of $3500 needs to be <br />supported by a breakdown of the costs associated with the <br />different reclamation tasks. This should be calculated for <br />the point of maximum disturbance during the operation. <br />Please see Rules 6.3.4 (1)(f), 6.3.4 (2), 6.3.4 (3), and <br />6.3.4 (4). As the application reads now, the Division will <br />have to require a higher bond.
