Long <br />Division Director <br />The Division has completed a preliminary adequacy review of the permit application noted above. <br />Please respond to the following comments and concerns by May 24, 1999 so the decision for <br />this application can be made prior to June 3, 1999. The decision must be made by this date to <br />provide an adequate window for apre-hearing conference that must be held at least ten days <br />after the decision and at {east five working days prior to the Formal Public Hearing. If you <br />cannot respond by May 24, 1999, please request a decision date extension. Please be aware <br />that failure to adequately address these comments and concerns, or submit a decision date <br />extension request, by May 24, 1999, could result in denial of this application. <br />ADEQUACY COMMENTS <br />1 . Item No. 7.1 on the application form requests the names of any parties that have recorded <br />easements on the affected land. Please indicate whether Public Service Co. or any other <br />company have utilities easements across the proposed affected area. <br />2. Item no. 9 on the application form needs to have the location marked to the Quarter/Quarter <br />section. Please revise this page of the application accordingly. <br />3. Rule 1.4.7 (5) requires a statement that the applicant has filed for all necessary local <br />government approvals. Please include this statement in Exhibit M. <br />4. Topsoil stockpiles are to be seeded, or otherwise stabilized, if they are to be in place for over <br />one season. please indicate if a temporary seed mix will be used for this purpose or indicate <br />which permanent seed mix will be used. If a temporary seed mix is to be used, please <br />provide a list of the species it will include and an application rate for each species. <br />5. The mining plan indicates that facilities for crushing, screening and washing will be used on <br />site. Please indicate if these facilities will require foundations. If so, please indicate what <br />tasks will be necessary to remove these foundations for site reclamation. <br />6. There appear to be several consumptive water uses associated with this gravel pit both <br />during and after mining operations. During mining -evaporative loss, dust control on roads <br />
