PART I <br />Page 2 <br />• Permit No. COG-850000 <br />A. COVERAGE UNDER THZS PERMIT <br />1. This general permit authorizes discharges that consist entirely of <br />precipitation-induced surface runoff from coal mining and processing <br />operations and loadouta, while active mining ie taking place or during <br />reclamation, into waters of the State of Colorado. Sxceptioas era those <br />facilities i.[t the Cherry Creek Basin upstream of Chatty Cr~slc F~s~rvoir <br />and those tributary to Chatfield Reservoir downstream of the USGS gage at <br />Waterton or in the Plum Creek drainage. No mine water, preparation plant <br />water, spoils spring water, domestic sewage or other wastewater other <br />than surface runoff may be discharged. Multiple discharges lram one <br />facility may be authorised through this permit but ALL DISC811RG83 FROM A <br />FACILITY COVERED BY THIS PERMIT MOST CONSIST ENTIRELY OF SURFACE RONOPF. <br />If a facility has discharges from one or more of the above ezcluded <br />sources alone or in addition to surface runoff discharges, an individual <br />permit is required. <br />2. In order to apply for certification under this general permit, the owner, <br />operator, and/or authorized agent of the subject facility shall submit by <br />certified mail or hand delivery, the completed Coal Mining Wastewater <br />Discharge Application (furnished by the Division) which generally <br />requires the following information (at a minimum): <br />• a) Name, address and descriptive location of the facility; <br />b) Name of principal in charge of operations; <br />c) Name of the water receiving the discharge; <br />d) Description of the activity including the anticipated duration of <br /> activity and/or discharge, anticipated volume, and rate of <br /> discharge; <br />e) Description of any waste water containment and/or treatment; <br />f) A map showing the general geographical location of the facility and <br /> a sketch of the facility showing outfall(s) and receiving water(s); <br /> and <br />g) An analysis of the water to be discharged. <br />h) Certification of completion of a Stormwater Management Plan (SWMP). <br />At least thirty (30) days prior to the anticipated date of.discharge, <br />such information shall be submitted to: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The Division shall have up to thirty (30) days after receipt of the above <br />information to request additional data or deny the authorization to <br />discharge. Upon receipt of additional information the Division shall <br />have an additional 30 days to issue or deny authorization to discharge. <br /> <br />Amended LO/L3/94 Effective Ll/13/94 <br />
