Indicate the type of operations at the facility; sand 6 gravel mining, <br />washing, and/or crushing. List principal products and months of <br />operation. Please specify what type of discharge, if any, will be <br />associated with each operation; pit dewatering, wash xaters, transport <br />waters, or other processes used. Sand washing, mainly during <br />summer mon*_hs. Water from deep well will be used to wash sand <br />extracted from dry creek bed. Water will discharge to settling pond <br />6. Do you anticipate the need for changing or adding the discharge point(s) <br />as mining progresses? If so, briefly explain. vA~ ~ a ran ~o; l l r,A m; nAd <br />in 10 acre phases. Se*_*_ling pond may be moved with each phase. <br />There will be no direct discharge into creek bed. <br />7. A location map designating the facility property and discharge points <br />shall be submitted. The map shall be from a 7-1/2 or 15 minute USGS <br />quad sheet or a map of comparable scale. A north arrow shall be shown. <br />Any public water supply intakes within a 5 mile downstream of the <br />facility shall also be identified, if known. <br />8. A legible general sketch of the site shall be submitted and include <br />appurtenant facilities (buildings, ponds, diversion ditches, stockpiles, <br />etc.), stream location, numbered discharge points, sampling and flow <br />monitoring points. Sketch shall be on paper 8 1/2 x 11 inches, Please <br />label the outfalls on the sketch to correspond with numbers Listed in <br />Table 9A. See MLRB Application Exhibit C - PreMining and Mining Plan <br />Map attached. <br />9. Do you have bulk storage of diesel fuel, gasoline solvents or other <br />hazardous materials on site? YES x NO Zf yes, then <br />indicate location on general sketch map (8). <br />10. Optional: You may attach additional sheets describing any additional <br />water pollution control programs (such as Mined Land Reclamation <br />Stipulations) which may affect your discharge. <br />A copy of the mining and excavation plan submitted to the MLRB <br />and the County is attached. <br />-4- <br />
